Because the first building (two, actually) was located at Madison Sq. They kept the name after they moved it. The one above Penn Station is the 4th iteration.

The avenues for sure but the side streets, the streets most people live on, are not as large. And it doesn't make it any less walkable.

IDK if this has been brought up but I was just looking in detail at the proposed roads and they all strike me as overkill. It looks like the Seaport with even the side "streets" having 4 or 5 lanes...

Keeping this capped at 6 floors is such a waste. Not saying there should be some 20 story tower here but this is prime real estate. No wonder Boston has become so expensive.

Because that costs more over time. This is a better option, especially if Everett is going to expand their BRT program which will be a far cheaper option for improving transit.
